The Exemplification will be held at St. Sebastian Church 155 Washington Street Church Middletown. \nThe honoree will be Sir Knight Antonio “Buddy” Hernandez\, post mortem. \nInformation on the Exemplification: \n                   Dress for Candidates and Special Dignitaries of the 4th Degree: \nCandidates will wear a dark business suit and conservative tie. Note that active members of the Armed Forces\, Police or Fire Personnel may wear their Class A uniforms. 4th Degree Sir Knights may wear 4thDegree uniform\, a tuxedo or dark suit with the social baldric. \nCosts: \n       Candidate Fee:                       $60.00 (Includes Lunch) \n         Sir Knight Guests                  $25.00 (Lunch) \nTentative Schedule: (which may be slightly adjusted as the Exemplification nears) \n        10:00-11:00 AM      Registration \n 11:30 AM                    Exemplification \n        1:30 PM                       Lunch \n There will be NO LADIES PROGRAM OR BANQUET for this Exemplification. All guests must be 4th Degree Knights. Priests are guests of the District Master and do not pay a candidate fee. \nAttached is the required form for registering candidates and guests for the Exemplification. This form\, along with properly completed Form 4’s and full payments (payable to: Chris Randall\, District Master) should be submitted to SK Michael DeConti 15 Pilgrim Path\, Farmington\, CT 06032 \nAssemblies are to submit all 4 copies of the Form 4 with all required signatures. The Form 4 will be signed and dated by the District Master then appropriate copies will be returned to the Assembly Comptroller after the Exemplification. Candidates and Sir Knights should make their check out to the Assembly so the Assembly can submit 1 check to the District Master. Please note that the Candidate letter will also provide access to a fillable online Form 4 that they may fill out\, sign\, print and give to the FS for verification of 3rd Degree member in good standing. DESCRIPTION:The Knights of Columbus invite you to coffee and after St Margaret’s 10:30 Mass.  Coffee\, juice and water along with baked goods are provided. \nCoffee Grounds is a great way to meet other members of our parish and introduce them to the Knights of Columbus. \nMembers of the Knights\, please stop down after 8:30 mass or arrive early for the 10:30 mass to help with set up.  It would also be appreciated if some members could remain after to help with cleanup.
